For your own privacy, and to increase efficiency please wear a short-sleeved shirt or an item of clothing that allows easy access to your upper arm / shoulder area.

To help this process run smoothly, please be aware of the following information:

  • Fill out your consent form in advance and bring it with you
Download the flu vaccine consent form (DOCX)
  • We know that sometimes people forget their form and we will have forms available but please have your NHI number with you or your University ID or driver licence
  • If you do not have an NHI number, let the clinic know when you arrive for your appointment
  • You must wait for 5 minutes after being vaccinated before leaving the clinic (20 minutes if this is your first flu vaccination)
