Seeking participants with mental health or addiction experience for a survey on physical health
Are you
- Aged 18 or over and
- Currently seeing a healthcare provider for your mental health or addiction
If so, we invite you to take part in a quick survey, aimed at examining your experience of physical healthcare in Aotearoa.
About the survey
This survey is about experiences of physical health and healthcare in Aotearoa. We would like to use this survey to identify ways to help people with experience of mental illness in the future, and to help with planning both physical and mental health services.
The survey takes around 20 minutes to complete. Please note:
- Your participation is entirely voluntary (your choice)
- You do not have to answer every question
- You may withdraw from the survey at any time
- You may ask someone you trust to help you fill in this survey, if you wish
What about my private information?
The survey is confidential to the study researchers only. Unless you choose to add your name at the end to be contacted further, the survey will be entirely anonymous.
This study has received ethical approval from Southern Health and Disability Ethics Commitee and was designed by research staff at the University of Otago.
